Initial report says driver error to blame in deadly Irving bus crash

Investigators say the deadly bus crash in Irving earlier this month appears to have been caused by driver error.

Three people were killed and dozens injured when the bus veered off and flipped on its side on Highway 161.

The driver of the bus told investigators he may have blacked out moments before the bus crashed.

This is the first report on a possible cause since the accident a week and a half ago. It's still not clear why the driver lost control.

The report does not indicate any kind of mechanical problem, like a tire blowout. Some on board the bus had speculated that was the cause.

A final report is still months away from being completed.

The charter bus was carrying senior citizens on a trip to a Casino in Oklahoma.
