Report: Cory Monteith died of drug-and-alcohol combo

"Glee" star Cory Monteith died from a deadly combination of alcohol and drugs, according to authorities in Canada, TMZ reported.

An autopsy and toxicological analysis show that the 31-year-old admitted addict died on from a "mixed drug toxicity, involving heroin and alcohol," according to the British Columbia Coroners Service.

Monteith was found dead inside his hotel room in Vancouver on July 13 after being out very late.

Monteith was in rehab earlier this year.
