New Developments In Day-Care Death

FOX 29's Jeff Cole reports a developing story surrounding a seven-year-old boy who drowned at a Philadelphia swimming pool. There's a scathing new report about the state's role in the incident.

The report is 77 pages thick and documents what investigators say was mistake after mistake that contributed to the drowning death of Isear Jeffcoat. His mother Tamisha Madalion spoke emotionally about her son earlier this year.

"He was like the man of my house. He liked to carry heavy bags for me and he was a curious little boy," Madalion said.

The child's body was found nine feet down in a pool owned by the mother of his daycare provider Tianna Edwards.

Investigators found Edwards, the owner of Tianna's Terrific Tots on Rising Sun Ave., had a criminal history which should have barred her from holding a daycare license, but used the name of a family member to open two facilities.

The report released by the state Tuesday found Edwards' daycare centers, over nearly 3 years, amassed 69 violations most of which were found during inspections when Edwards knew regulators were coming.

It took state inspectors 3 months to investigate claims Edwards was using a false identity on her daycare license, and then didn't look at the picture handed over to prove identification.

The state is pledging reforms while Tianna Edwards is in jail awaiting trial on manslaughter charges in Isear Jeffcoat's death.

"I feel like she was not who she said she was. I would have never placed my child there if I knew the type of person she was," Madalion said.

The Department of Public Welfare said it's stepping up both "unannounced and announced visits to daycare centers among other reforms.

On Wedneday, Jeffcoat's mother said in a statement "My son didn't need to die, if they had just done their job."

Her attorney Nancy Winkler said, There was an absolute breakdown in the system" and called for "meaningful changes" so this won't happen to other children.
